We Welcome New Patients!
Thank you for choosing Glendale Dental Arts. We appreciate your confidence in our office! Be sure to read about our history and our committment to excellence if you haven't yet.
As a new patient,
we welcome you to call our office to set up your initial
appointment.
Our friendly secretaries will help find the best
time for you and explain what to expect on your first visit.
They will also ask you a few questions about your insurance so that
we can save you time by being prepared with eligibility information
before you arrive.
When you come in for your first visit, there is a single required form to be filled out, the Patient History form. There is also some required reading, and some optional reading. You will be provided everything you need in person. However, if you prefer to save time, you can start right now! Everything you need is on our forms page. Start with the required reading. Then print out the double-sided Patient History form and fill it in. Finish with the optional reading if you are interested.
Again, thank you for choosing Glendale Dental Arts.

What to Expect During Your First Visit
During your initial visit, you will be
introduced to our office and staff. If you like, you can have
a short tour of the office, including the sterilization room and
equipment. Getting to know our office will help you feel more
comfortable, arguably even "at home" when you visit us.
You will need to fill out a Patient History
form if you haven't already.
If you have any dental insurance, your
eligibility will need to be verified. Please have the information ready when you first call our office. We will need
the member's employer, the insurance plan name, and information about you and the member (e.g., your spouse, your parent) such as
date of birth and SSN. Many times we are able
to do this before you arrive in our office, but in some cases we
will do this for you while you are filling out the form.
The next step is to have x-rays taken. We will ask you questions to determine which x-rays
need to be taken and how many. We may take some individual
x-rays, a full mouth series, or a digital
panorex depending on your needs. If you have copies of recent
x-rays from another office, you may bring them with you. Be warned though, most copies of x-rays are not clear enough to be very useful. You may still need new x-rays taken.
When the x-rays are ready, the doctor will
examine them with you and discuss your dental situation with
you. Depending on the complexity of your dental needs, this
may take as little as 2 minutes, or as much as 30 minutes or more. You may
ask all those little questions that have been bouncing around in
your head and your doctor will be happy to answer them for you.
If you do need dental treatment, you can then
discuss your treatment plan with our financial manager who will help
explain everything about cost and insurance coverage. She will
also show you models and help clarify all the dental options you
have. This can take as little as 5 minutes, or for complex
cases, much longer.
About Treatment During Your First Visit
Unless you have a dental
emergency, you should
not expect any treatment during your first visit due to time
constraints, not even cleanings. Anything and everything you
need will be scheduled for later visits.
